A device history record review could not be performed because a lot number was not received with the complaint. As part of our manufacturing process, all device history records are reviewed and approved by quality, prior to release of product.A physical sample was not returned for evaluation. However, one digital photograph was provided for analysis and investigation. The catheter shown in the photograph was inside a generic plastic bag and indicated signs of use (blood). It was observed that the catheter was broken in two/cut.An ishikawa diagram was used to determine the potential causes for this event.Based on the available information, the most probable root cause is due to instruments with sharp or rough edges during clinical use resulting in a broken catheter. It is important to consider that the instructions for use state: exercise caution when using sharp instruments near the catheter.Do not use instruments with sharp or rough edges directly on the catheter since even a minor cut break the catheter. It must be noted that in-process controls (such as personnel training, incoming quality acceptance testing for raw material, 100% in process visual inspection and visual acceptance sampling are performed in the plant) are in place to prevent nonconforming product from leaving the manufacturing operations. This complaint will be used for tracking and trending purposes.
