Based on the on-site evaluation, the field service engineer observed the detergent nozzle was leaking water.Cleaned and tightened detergent nozzle.During repair, found that the front door was not shutting properly and replaced door latch.Noted that the push plate was cracked.Replaced the push plate.While removing the lid to replace the push plate, found that the lid was cracked.Replaced the lid.Equipment repaired and verified according to oem instructions.Software attributes have been verified and confirmed.Equipment passed the electrical safety test and results.The cause could be traced to maintenance.No further information was reported.
 
Event Description
The customer reported to olympus there was fluid leakage inside the device.There was no patient injury reported.
 
Manufacturer Narrative
The investigation has been completed.A device history record (dhr) review was performed and found no non-conformances that would cause the reported malfunction.All records indicate that the device was manufactured in accordance with all applicable procedures.The instructions for use (ifu) states: ¿before using the equipment, always check that there is no irregularity regarding the following points on the lid and the lid packing.If there is any irregularity, cleaning fluid or disinfectant solution may leak out.¿ the root cause could not be conclusively determined.Probable causes that could have led to the reported event include due to the user handling such as the user dropped an object on the lid.
