Additional Manufacturer Narrative
This report is being submitted as follow up no.1 to update section h3 and h6, and to provide the completed investigation results. The codes were unable to be selected in the initial report due to an application error; therefore, section h6: patient code has been provided. One 6fr destination sheath, dilator, packaging card and packaging pouch were received for product evaluation.Visual inspection revealed that the dilator and the sheath were returned inside the pouch, nested in the packaging card, and it was noted that the pouch was slightly opened. Two folds were seen on the packaging card at 33 cm and 53.1 cm from the proximal end of the tray. There were kinks noted on the sheath and dilator near the folds on the packaging card. The kink noted on the sheath was 44.1 cm from the sheath hub.The kink noted on the dilator was 58 cm from the dilator hub.Based on the provided information and investigation results, there is no definitive evidence that this event was related to a device defect or malfunction. It is likely that mishandling of the device and the packaging may have contributed to the damage noted on the sheath and dilator. However, the exact cause of the reported event cannot be definitively determined based on the available information.
