(b)(4).Insulin pump received with broken reservoir tube lip, missing reservoir tube o ring and cracked battery tube threads.A test reservoir was installed and did not lock in place due to complete broken reservoir tube lip.
Event or Problem Description
Information received by medtronic indicated that half of the reservoir compartment entrance broke off.Customer reported that there was physical damage to the insulin pump's retainer ring and the reservoir was able to lock in place when inserted into insulin pump.No harm requiring medical intervention was reported.The device will be returned for analysis.
