Event or Problem Description
It was reported that the procedure was to treat a renal stenosis with balloon angioplasty only. The 5.00x12 mm trek balloon dilatation catheter (bdc) was inflated multiple times, 8 atm for 15 seconds, 14 atm for 20 and 30 and 15 seconds. There were no issues with inflation or deflation of the balloon; however, it was not noted how long negative pressure was held before attempting removal of the bdc.During removal of the trek bdc, resistance was felt with the guiding catheter and it caused a portion of the balloon to shear off.Snare was attempted but was unsuccessful. The portion remains in the patient. The patient is doing fine and was discharged. There was no reported clinically significant delay in the procedure. No additional information was provided.

Additional Manufacturer Narrative

A visual inspection was performed on the returned device and the reported balloon separation was confirmed. The reported difficulty to remove through the guiding catheter was unable to be confirmed due to the returned condition of the device.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other similar incidents reported from this lot.In this case, the device was visually inspected and prepped prior to use without any issue noted, which would suggest that the device was not damaged prior to use.The investigation determined the reported difficulty to remove, balloon separation and noted damages appear to be related to operational circumstances of the procedure. In this case, it is likely that the balloon was not given enough time to deflate causing the difficulty to remove through the guiding catheter.Manipulation against resistance ultimately resulted in the balloon separation and subsequent damages.Additionally, the treatment appears to be related to the operational context of the procedure as unsuccessful attempts were made to remove the separated portion of the balloon and the separated portion remains in the patient. There is no indication of a product quality issue with respect to manufacture, design or labeling.
