Event Description
The customer reported a ventilator that would not run on ac, and only on battery.The manufacturer's field service engineer confirmed the reported problem.There was no patient involvement or harm.The manufacturer's field service engineer replaced the power supply to address and correct this reported event.
 
Manufacturer Narrative
G4: 20oct2020.B4: 26oct2020.V60 power supply was received for failure evaluation.Visual inspection revealed no evidence of damage or contamination.A failure investigation (fi) technician installed the v60 power supply into a fi ventilator to duplicate the reported problem.During the unit testing, the fi technician identified that q1 (500v single n-channel hexfet power mosfet) leads 1-3 shorted and cr5 (turbo 2 ultrafast high voltage rectifier) leads 1-2 shorted.The customer complaint was verified.Submission of a report does not constitute an admission that medical personnel, user facility, importer, distributor, manufacturer, or product caused or contributed to the event.
