Event Description
|
According to the reporter, they had a capsule which failed to attach.The capsule fell off and lodged at the airway as the capsule was being removed via snare from the patient without complications.Upon endoscopic verification, the capsule was found resting next to the vocal cords.They stated that the delivery device was not as straight out of the package as the capsule that was attached and the delivery device was slightly bent.They used another capsule to complete the procedure after the first capsule was retrieved.There was no harm to the user, there was nothing unusual about the patient or the procedure, and an endoscopy had been performed prior to the procedure and showed the esophagus to be normal.A little bit of lubricant was used to facilitate placement of the capsule and the delivery system and capsule will be returned for investigation.
