Event Description
It was reported that during the left ventricular (lv) lead placement in the coronary sinus (cs), more than ten different catheters/wires were used to cannulate the cs and maintain access.A dissection took place when using the last catheter.The lv lead subsequently could not be placed.No further patient complications have been reported as a result of this event.
