Event Description
It was reported that shaft break occurred.A 2.50mm x 20mm maverick balloon catheter was advanced for pre-dilation.However, the balloon delivery shaft was not firm and the device was fractured during repeated dilation and advancement.The device was retreated into the guide catheter and removed from the patient's body.No other information available.
