Event or Problem Description
One resolute onyx des was attempted to be used to treat a moderately tortuous, moderately calcified lesion with 80% stenosis in the mid lad. There was no damage noted to the device packaging. There were no issues noted when removing the device from the hoop. The device was inspected with no issues. The lesion was pre-dilated.The device did not pass through a previously deployed stent.Resistance was encountered when advancing the device but excessive force was not used.It was reported that stent deformation occurred when attempting to remove the stent from the artery. The stent deformation occurred at the tip of the guide catheter. It was stated that the stent would not go into the tip of the guide catheter, so the physician attempted to pull the guide all the way back to the sheath in the right wrist. The deformed stent would not go in the sheath so the physician had to do a cut down on the radial artery to get the stent out.It was stated that at this point the stent was off the balloon and a hemostat was used to remove the stent, causing it to elongate. No further patient injury reported.

Additional Manufacturer Narrative
Additional information: the stent failed to cross the lesion. No difficulties were noted when removing the protective sheath. The stent was not inspected post-removal of the sheath. The procedure was completed the following day.The sheath was non-medtronic.Medtronic is submitting this report to comply with fda reporting regulations under 21 cfr parts 4 and 803.This report is based upon information obtained by medtronic, which the company may not have been able to fully investigate or verify prior to the date the report was required by the fda.Medtronic has made reasonable efforts to obtain more complete information and has provided as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report. In particular, this report does not constitute an admission by anyone that the product described in this report has any ¿defects¿ or has ¿malfunctioned¿.These words are included in the fda 3500a form and are fixed items for selection created by the fda to categorize the type of event solely for the purpose of regulatory reporting.Medtronic objects to the use of these words and others like them because of the lack of definition and the connotations implied by these terms. This statement should be included with any information or report disclosed to the public under the freedom of information act.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.A good faith effort will be made to obtain the applicable information relevant to the report. If information is provided in the future, a supplemental report will be issued.

Additional Manufacturer Narrative
Image review: angiographic images provided confirmed a calcified vessel with 80% stenosis in the mid lad. There appeared to be a previously deployed stent from the left main to the target lesion in the lad.The lesion was immediately distal to the previously deployed stent.A wire was delivered to the vessel.A balloon was delivered to the target lesion and expanded in the vessel. The balloon does not appear to have concentrically expanded, confirming the difficult target lesion morphology. There were no images provided showing the reported resistance encountered, failure to cross, deformation and dislodgement of the stent or removal of the dislodged stent.Medtronic is submitting this report to comply with fda reporting regulations under 21 cfr parts 4 and 803.This report is based upon information obtained by medtronic, which the company may not have been able to fully investigate or verify prior to the date the report was required by the fda.Medtronic has made reasonable efforts to obtain more complete information and has provided as much relevant information as is available to the company as of the submission date of this report.
