Event or Problem Description

Additional information: d4 - lot-number; d10 - concomitant medical products and therapy dates; h4 - device manufacturer date.Correction: d1 - brand name; d2 common device name, device product code; d4 - material number, catalog number; g4 - pma/510(k) number.Device evaluation: the suspect medical device was not returned to the manufacturer for investigation but to olympus medical systems corporation (omsc), japan (returned to omsc on (b)(6) 2020).The investigation confirmed severe corrosion in the electrode¿s locking element and on the instrument¿s surface. This was most likely caused by incorrect reprocessing.Furthermore, the internal electrode fixing port was found to be blocked by a torn-off electrode contact. This type of damage is typical for excessive force. Therefore, this event/incident was attributed to use error.A material or quality problem can be excluded since a manufacturing and quality control review was performed for the affected lot number of the working element without showing any abnormalities. The case will be closed on olympus side with no further actions. However, the reported phenomenon will be recorded for trending and surveillance purposes and the user will be informed about the investigation results.
