Event Description
|
During a procedure, an attempt was made to use one sprinter legend rx ptca balloon catheter to treat a lesion.There was no damage noted to the packaging.There was no issues noted when removing the device from the hoop.The device was inspected with issues noted.It was reported that the device cracked at the luer / hub when applying pressure via the inflation device.It was stated that a leak occurred.The patient was reported to be alive with no injury.
