Event Description
The following information is alleged: the patient received a gunther tulip on (b)(6) 2007.Approximately 11 years and 7 months later, it was discovered that the filter had multiple fractured filter struts.Hospital and medical records have been requested, but not yet provided.

Event Description
Patient allegedly received an implant via the right internal jugular vein due to bilateral pulmonary thromboembolism (pte).Patient is alleging device fracture.Patient notes and further alleges experiencing "constant pain on a daily basis making it hard to work.I am unable to stand for long periods of time because of the physical pain", physical limitations, and depression.(b)(6) 2019 computed tomography (ct) abdomen without contrast: "impression: aberrant ivc filter placement, terminating at the level of the diaphragm hiatus.Multiple fractured struts suggested".
