Insulin pump received with broken belt clip slot, broken reservoir tube lip, missing reservoir tube o ring and cracked battery tube threads.A test reservoir was installed and did not lock in place due to complete broken reservoir tube lip.(b)(4).Currently it is unknown whether or not the device may have caused or contributed to the event as no product has been returned.No conclusion can be drawn at this time.We therefore consider this report complete to the best of our knowledge.
Event or Problem Description
Information received by medtronic indicated that the insulin pump was chipped off.Customer stated that the belt clip area was chipped.No harm requiring medical intervention was reported.The insulin pump will be returned for analysis.
