Complaint internal reference: (b)(4).H10, h2, h3, h6: the reported device, used in treatment, was returned for evaluation. There was a relationship found between the reported incident and the returned device.A review of the device history records for the device showed there were no indications to suggest that the product did not meet manufacturing specification or would not be able to perform as intended.A review of risk management files found that the reported failure was documented appropriately.A complaint history review concluded this was a repeat issue.A review of the instructions for use found: please read the controller user's manual prior to using this device. The controller and the wands are designed to be operated as a system.Prior to initial use, ensure that all package inserts, including warnings, precautions, user¿s manual, and instructions for use are read and understood.Do not place other cables between the integrated cable component and the controller.Visual inspection under magnification shows minimal erosion on the electrodes. There are bent pins observed inside the connector. The device could not be connected to a controller and tested due to damaged pins. The complaint was confirmed.Factors that could have contributed to the reported event include: (1) there may have been misalignment of the connector when connecting to a controller in which excessive force could cause damage to the pins.(2) twisting the connector during connection / disconnection to a controller which could have caused pin damage. There were no indications that would suggest that the device did not meet product specifications upon release into distribution.
