Event Description
Information was received from a healthcare professional (hcp) via company rep regarding a patient receiving gablofen (500mcg/ml at 1 81.94mcg/day) via intrathecal infusion pump for intractable spasticity.It was reported that during pump replacement the pump locked while the scrub nurse was filling the reservoir with medication.They were unable to push the full amount of med in due to push back from the pump.It was reported that there was possibility of air getting into the pump during med fill.No diagnostics were performed.Withdrawal of the med that was in the pump and a 20 min warm sterile bath of the pump was recommended.Once the recommendations were completed the pump was able to be filled as normal.The issue was resolved at the time of the report.The patient¿s status was alive with no injuries.No symptoms were reported.

Event Description
Additional information was received from a company rep and confirmed by a physician.The patient's weight was reported.It was reported that symptoms weren't applicable.It was reported that there wasn't a way to determine the exact cause of the pump locking up.It was assumed that air got in while the pump was being prepped (withdrawing the sterile water and adding the medication).It was reported that the explanted pump was replaced due to noraml eri/eos.
