Event Description
It was reported that "today the first day of the training was held and it is possible to identify that they are in serious trouble locating the tip of the catheter in the chest x-ray, due to the visibility of the institution's radiography equipment.They had a serious adverse event, where the insertion of the catheter was performed in the neonatology unit without confirmation of the tip of the catheter, and then was released for use, later the client presented cardiac tamponade, it was verified by contrast x-ray that the catheter was positioned in the right ventricle.".
