Medtronic received a report that the physician attempted to deploy the pipeline, but after about 70% of the device was deployed, a twist occurred.
Resheathing and redeployment was done four times, but the pipeline was still twisted and not opened in the middle section.
The pipeline was then removed, and a replacement pipeline was implanted successfully.
It was indicated that all devices were prepared as per the instructions for use (ifu), and no patient symptoms or further complications were reported as a result of this event.
