Internal reference: (b)(4).This case was reviewed and investigated according to the manufacturer¿s policy.Block h3: visual and microscopic inspection were performed on the returned device. The device was returned in two separate pieces. The entirety of the device was returned. The distal coil was stretched. There are no missing components and no rough/sharp edges except where the core wire was exposed. The probable cause of the reported ¿the wire "looked different" when removed from packaging¿ failure could not be determined by this investigation. The probable cause of the reported ¿wire snapped, bent and got stuck¿ failure is damage in use due to, an inappropriate manipulation.Strain on the wire can cause damage to internal components and can result the separation and/or stretching of the distal coil.As the customer confirmed the device was in one piece after removal from the patient, the probable cause of the observed separated distal coil and separated/exposed core wire damage likely occurred during preparation of the device, or in transit for return.Submission of this report does not, in itself, represent a conclusion by the manufacturer and/or authorized representative or the national competent authority that the content of this report is complete or accurate, that the medical device(s) listed failed in any manner and/or that the medical device(s) caused or contributed to the alleged death or deterioration in the state of the health of any person.
