(b)(4).Pump received with broken reservoir tube lip, missing reservoir tube o ring and cracked battery tube threads.A test reservoir was installed and did not lock in place due to complete broken reservoir tube lip.Currently it is unknown whether or not the device may have caused or contributed to the event as no product has been returned.The device will be returned for analysis and further information will follow once the analysis has been completed.No conclusion can be drawn at this time.
Event or Problem Description
Information received by medtronic stated that the retainer ring was damaged.Customer stated that they were able to lock in place the reservoir when inserted in insulin pump.No harm was required during medical intervention.The insulin pump will not be returned for analysis.
