Additional information provided in d.10, h.3, h.6 and h.10.A review of the device history record traceable to the reported lot number indicates that the product was processed and released according to the product¿s acceptance criteria.One wet intrepid plus fms was visually inspected and functionally tested.It was tested on a calibrated console and failed to prime, generated system message code 162.Leakage was observed from the aspiration tubing insertion to the cassette.Visual inspection indicated more solvent than typically applied around the tubing was present on the wall of the aspiration port of the cassette.The root cause of the customer's reported event was due to an assembly error during the internal manufacturing process.Due to most likely improperly applied solvent used during the assembly process, the aspiration tubing did not properly adhere to the cassette, which caused the priming failure.This complaint has been reviewed and it is determined that no further actions will be pursued at this time.In-process controls are established to ensure each final assembled cassette is verified that all required tests have been performed and all acceptance criteria are met prior to release.A complaint trend review for this lot also indicated this to be an isolated event for this finished goods lot.Quality assurance has reviewed this complaint and will continue to monitor data for evidence of adverse trending and take further action, as appropriate.The manufacturer internal reference number is: (b)(4).
