Evaluation summary: medtronic conducted an investigation based upon all information received. The device and a photo were available for evaluation.Visual inspection noted the handle's quick connect assembly was disengaged.Scratches were noted on the inner wall of the collar, indicating that the collar and ball ramp were pressed together. The ball ramp was inspected and no visual deformation or imperfections were seen.It was reported that a component disengaged from the device. The reported issue was confirmed. The most likely cause was determined to be manufacturing related. This issue can occur if there was an out of specification part in combination with exposure to heat. This will cause a defect outside of the operating room during autoclaving, with no proximity to the patient. The condition will not cause any patient harm. When the collar is still in position on the ring, it cannot separate under normal operating conditions. If information is provided in the future, a supplemental report will be issued.
