The system was serviced in the field and passed all tests.No failures were found.No registration data was available on the archive sent in.Multiple patient trackers and all the registration types were tried, but no registration data appeared.Troubleshooting was done.The em interface showed no fault and the emitter shows tracking on all coils.The representative (rep) was able to test in an or setting and was able to successfully verify all four instrument trays.Likely cause for inaccuracy was user error or poor scan quality.If information is provided in the future, a supplemental report will be issued.
 
Event Description
Medtronic received information regarding a navigation device being used intra/peri-operatively of a functional endoscopic sinus surgery (fess) procedure.It was reported that, while navigating, the surgeon felt inaccurate.The surgeon registered 4 times hoping to increase accuracy without any success.Each time the registration appeared to be accurate on the surface, the surgeon felt inaccurate with the instrument.The site felt off with all the instrument and decided to abandon navigation.There was a 10-minute delay to the procedure.Additional information was received.It was reported that the likely cause for the inaccuracy was user error or poor scan quality.The inaccuracy was approximately 10mm.
