(b)(4).Insulin pump received with blank display and constant tone due to moisture damage on the electronics assembly.Unable to perform all functional testing including the operating currents, unexpected restart error test, rewind, basic occlusion, occlusion, prime, compromised force sensor system and excessive no delivery test due to blank display.Pump received with moisture damage motor, vibrator and battery tube assembly.Pump received with cracked battery tube threads, cracked reservoir tube lip and cracked case reservoir tube window corner.The p-cap locks into the reservoir compartment properly noted.
Event or Problem Description
Information received by medtronic indicated that the insulin pump had insulin pump error alarm.Customer was able to clear alarm.Customer was able to complete rewind.No harm requiring medical intervention was reported.The insulin pump will be returned for analysis.
