Event or Problem Description
It was reported that balloon rupture occurred.The target lesion was located in the left anterior descending artery.A 3.0mm x 15mm quantum maverick balloon catheter was advanced for use.However, during procedure, the balloon ruptured when the pressure was applied.The device was completely removed and the procedure was completed with another of the same device.There were no patient complications reported and the patient was stable.

E1: initial reporter facility name: (b)(6) hospital.Device evaluated by mfr: returned product consisted of a quantum maverick balloon catheter.The shaft, hypotube, tip and balloon were microscopically and visually examined.There was contrast in the inflation lumen and balloon.The balloon was loosely folded.The device was prepped with an inflation device filled with water and connected to the inflation port to inflate the device.There were pinholes 5mm distal of the proximal marker band on both sides of the balloon.Inspection of the remainder of the device presented no other damage or irregularities.
