Event or Problem Description
|
|
It was reported that a patient underwent a ventricular tachycardia (vt) procedure with a navi-star¿ thermo-cool¿ electrophysiology catheter and there was no signal available for the physician to monitor the patient¿s heart rhythm.Initially it was reported that there was bad/partial signal interference (noise/loss) observed on both intracardiac and body surface channels.A second catheter was used to complete the procedure. There was no patient consequence reported.Attempts were made to obtain clarification to this complaint. However, with the information available, this signal issue was assessed as not mdr reportable as the risk to the patient was low for partial signal interference (noise/loss).Additional information was received on 9/22/2020 as it was clarified that the signal interference (noise/loss) was observed on intracardiac channels. The signal interference was seen on the carto 3 system only since no other equipment was used to monitor the patient¿s heart rhythm. There was no signal available for the physician to monitor the patient¿s heart rhythm.During the interference, the catheter was inside the patient¿s body.Since it was clarified that there was no signal available for the physician to monitor the patient¿s heart rhythm, this issue is reassessed to an mdr reportable malfunction. The awareness date for this issue is 9/22/2020.
