The guide wire was received at csi for analysis, engaged in the oad.Visual examination revealed two guide wire fragments in the driveshaft tip bushing.Scanning electron microscopy analysis revealed the two guidewire fragments had fractured due to torsional forces.The exact root cause of the fractures could not be determined.The material inspection report for the reported guide wire could not be reviewed, as the lot number was not provided.Csi id: (b)(4).
 
Event Description
During the procedure the orbital atherectomy device (oad) became stuck on the wire and both the wire and oad were removed together.There were no patient adverse effects.Failure analysis of the returned oad identified two guide wire fragments returned in the driveshaft.
