The subject device in this report has not been returned to omsc for evaluation.The exact cause of the reported event could not be conclusively determined at this time.If additional information becomes available, this report will be supplemented.
 
Event Description
Olympus medical systems corp.(omsc) was informed from the user that during the reprocessing of the subject device with the cleaning brush, the foreign material was found inside the channel.The user pushed out and flushed out the channel, the fragment of the rubber which was a part of the suction valve was came out from the subject device.The field service engineer of olympus thailand checked the subject device on site and found that there was no problem even water leakage.There was no report of patient injury associated with this event.
 
Manufacturer Narrative
This supplemental report is being submitted to provide additional information.The subject device has not been returned to olympus medical systems corp.(omsc) for evaluation.When the olympus field service engineering (fse) visited the user, he proposed to the user checking all accessories condition before using them to prevent from the similar event happening again.Olympus contacted to the site and found that the similar event had not been happened for the next 14 days and there is no problem on the device.Omsc reviewed the manufacture history (dhr) of the subject device and confirmed no irregularity.The exact cause of the reported event could not be conclusively determined.However, it was probably that the event occurred because the suction valve was damaged by physical stress and rubber fragments were pushed out by the cleaning brush.
