Additional Manufacturer Narrative
Block h6: device problem code 3009 captures the reportable event of stent positioning issue.Block h10: a wallflex biliary rx uncovered stent was received for analysis; the delivery system was not returned.Visual inspection was performed and there were no damages noted on the stent. The outer diameter (od) of the stent and the stent length were measured and were found to be within specification. The reported event of stent positioning issue was not confirmed as it is impossible to replicate the failure reported occurred during the procedure on the laboratory during analysis of the device.Additionally, the stent was returned in good condition and the measurements were within the specification.A labeling review was performed, and from the information available, this device was used per the instructions for use (ifu)/ product label.Additionally, stent positioning issue is noted within the ifu as a known potential adverse event related to the use of the device. Therefore, a review and analysis of all available information indicated the most probable cause is known inherent risk of device.A review of the manufacturing documentation for this device revealed that no anomalies or deviations related to the event occurred during manufacturing.
