Event Description
Medtronic received information regarding a navigation system used intra operatively for a functional endoscopic sinus surgery.It was reported that the site tried to load a patient exam disc on the navigation system, as well as the ent software, and got the message "unable to interpret header data".The postponed the surgery and rescheduled the navigation portion of the surgery for this case.There was non- serious tissue harm for the patient present and the patient will be exposed to a second surgery.
 
Event Description
It was reported that issue occurred during a fess procedure.The procedure was completed without the navigation, no harm came to the patient because of medtronic equipment.The case ended up canceling because of cardiac issues not because of the medtronic navigation system.
