A doctor reported that the phacoemulsification tip plugged during quadrant removal resulting in occlusion bells, high pitched vacuum tone, lack of suction and loss of phaco power.The phaco tip needed to be removed from the eye numerous times and cleared in a bowl of balanced salt solution.This was a dense cataract.The procedure was otherwise completed normally.The consumables were discarded.
