Medtronic received a report that the axium coil was the fourth coil to be implanted. However, after deploying, the coil failed to detach twice when using the instant detacher. The manual detachment method was performed, but the coil still failed to detach.Upon slowly retrieving the coil back through the sl-10 microcatheter, the coil detached inside the catheter. The microcatheter and col were then removed from the patient together.A new catheter and coil were then used to complete the procedure. It was indicated that all devices were prepared as per the instructions for use (ifu), and no patient symptoms or further complications were reported as a result of this event. The patient was undergoing surgery for treatment of a saccular, unruptured aneurysm at the a1 and a2 junction with a max diameter of 6 mm and a 3 mm neck diameter. It was noted the patient's blood flow and vessel tortuosity were normal.
