During a procedure an attempt was made to use one resolute integrity coronary drug eluting stent to treat a moderately tortuous, severely calcified lesion exhibiting 60-70% stenosis in the proximal right coronary artery (rca) and 99% in the distal right coronary artery (rca), the ostium and mid rca was also required to be treated. The device was inspected with no issues noted.Negative prep was not performed. The lesion was pre-dilated.The device did not pass through a previously deployed stent.Resistance was encountered when advancing the device.Excessive force was not used during delivery. It was reported that stent dislodgement occurred during removal following a failed delivery. The stent was not removed and was deployed in the right arm radial artery. It was stated that per fluoroscopy the stent appeared to have been compromised during initial advancement against severe diffuse calcification and an attempt to withdraw the stent due to no cross failed. It is possible that the proximal struts caught on the guide lumen and therefore making a withdrawal through the guide and sheath impossible. The patient was reported to be alive with no injuries.
