Date of event: date of event was approximated to (b)(6) 2020 as no specific event date was reported.Initial reporter's phone number: (b)(6).(b)(4).The device was not returned for analysis; however, a media inspection was performed of photos provided by the complainant and noted that the stent was partially deployed and the deployment suture created a loop on the end to the shaft.Additionally, the loops of the stent were noted to bent.No other issues were noted to the stent. The reported event of stent partially deployed and stent deployment suture knotted were confirmed. The investigation concluded that the reported events may likely be due to factors encountered during the procedure and/or the manner that the device was manipulated during deployment, could have contributed to the stent deployment suture loop on the tip of the catheter and therefore, the stent could not fully deploy. The reported broken stent deployment suture may have also contributed to the stent partial deployment.Additionally, the loops of the stent were bent; however, there is not sufficient information about what could be the cause for this failure. Therefore, a review and analysis of all available information indicated the most probable cause is adverse event related to procedure.A review of the manufacturing documentation for this device revealed that no anomalies or deviations related to the event occurred during manufacturing.A labeling review was performed and from the information available, this device was used per the instructions for use (ifu)/ product label.
