Medtronic received information regarding an axium coil pusher that broke during retrieval. The patient was being treated for an unruptured fusiform aneurysm of the left internal carotid aneurysm. The aneurysm max diameter was 4mm and the neck diameter was 2mm.Blood flow was normal.Vessel tortuosity was moderate. It was reported that all devices used were prepared per the instructions for use (ifu). The coil was fully deployed and successfully detached. When the pushwire was withdrawn, it was found that it had broken and the distal half was left within the blood vessel. There had been no friction during testing or delivering.Continuous flush was administered during the procedure. The break point was determined to be 50cm from the distal end.The physician was able to remove the broken segment by pushing the guidewire and the microcatheter. There were no patient symptoms associated with the event.
