The results of the investigation are inconclusive since the reported device was not returned for analysis.Based on the information received, the cause of the reported event could not be conclusively determined.The device history record for this oad lot number has been reviewed.No issues or discrepancies were noted during this review that would have contributed to the reported event.The device met material, assembly, and quality control requirements prior to distribution.(b)(4).
 
Event Description
A diamondback peripheral orbital atherectomy device (oad) was operated within a previously placed stent in the superficial femoral artery.The oad was difficult to remove, and upon removal tissue was observed around the crown.The stent was undamaged.Imaging was performed and no flow was observed inside the stent in the area treated by the oad.A balloon was inflated and an additional stent was placed to resolve the issue.The case was completed with a good end result and the patient was discharged.Per the opinion of the physician, the cause of the event was that the device was inadvertently placed subintimal within the in-stent restenosis.
