Event or Problem Description
|
|
On follow-up it was reported that the fault in the device occurs intermittently during the anterior cervical discectomy and fusion p rocedure. The malfunction was expressed in the significant differences between the intensity of the electrical stimulus that the user adjusts and the intensity that the device actually emits to the tissue (as measured by the system and as evidenced by the lack of response). The fault also exists when using alternative end equipment and computer. The fault appeared without any known event.Sometimes, the measured intensity was 0 and sometimes, the value was significantly lower than the set value. The customer replaced all the components of the system one by one until it was found that the problem was due to a controller failure. This includes the computer system, cables, pre-amplifier and stimulating box. There was a delay and back-up device used during the procedure.
