Date of event: (b)(6) 2020.Date of report: 25nov2020.
 
Event Description
It was reported that the ventilator had an alarm light emitting diode (led) failure.The patient was removed from the ventilator and placed on an alternate ventilator.The patient was not harmed or injured as a result of the reported event.The customer troubleshot with technical support and found the error code in the ventilator diagnostic report.The customer reported that the led did light.The customer was advised to replace the power switch overlay.Upon a follow up with technical support the customer reported that after replacing the power switch overlay the unit ran for about a minute and showed a 35 volt fail error code and will not turn on.The customer was advised to try a different power management board and if that did not resolve the issue to try a different power supply.The customer was also advised to check all of his service work as this issue did not occur before replacing the overlay.

G4:03dec2020, b4:08dec2020.The customer called back to technical support and reported that after replacing the touchscreen and power management board the issue was addressed.Although requested, no patient information was provided.Submission of a report does not constitute an admission that medical personnel, user facility, importer, distributor, manufacturer, or product caused or contributed to the event.
