During a procedure an attempt was made to use one resolute onyx rx coronary drug eluting stent to treat a moderately tortuous, severely calcified lesion located in the proximal right coronary artery (rca). The device was inspected with no issues noted.Negative prep was performed with no issues noted. The lesion was pre-dilated.Resistance was encountered when advancing the device.Excessive force was used during delivery. It was reported that two weeks after the resolute onyx stent was implanted the patient was brought back to try a new strategy as during the first procedure difficulty advancing other devices occurred e.G.Guidelines/balloons and it was only possible to stent the proximal section.A non-medtronic stent system was implanted inside the resolute onyx stent and the resolute onyx stent became damaged. It was stated that the original resolute onyx stent may have been compromised. The patient was reported to be alive with no injury.
