The subject device in this report has not been returned to omsc for evaluation.The exact cause of the reported event could not be conclusively determined at this time.If additional information becomes available, this report will be supplemented.
 
Event Description
Olympus medical systems corp.(omsc) was informed from the user that during the preparation for use, the front panel of the subject device intermittently froze.Other detailed information was not provided.There was no report of patient injury associated with the event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the device evaluation and the legal manufacturer's final investigation.The device was returned to an olympus repair center for evaluation.The reported issue (front panel of the subject device intermittently froze) was found to be caused by a failure of the cp board (printed circuit board).Based on the legal manufacturer's investigation, since the device was manufactured over 10 years ago, it is likely that the parts on the cp board deteriorated over time due to long-term repeated use, causing the printed circuit board failure.The cp board controls the front panel; therefore, it is likely that the front panel froze intermittently due to the failure of the cp board.A review of the device history record (dhr) found no issues that could have caused or contributed to the reported issue.
