A titan one touch release pump and two cylinders were received for evaluation.Examination and testing of the returned components revealed the presence of surface abrasion on the longer length pump exhaust tubing and inlet tubing of the pump.No visible abnormalities were noted with the pump or pump tubing. No functional abnormalities were noted with cylinder 1 and 2.The information received indicated a tubing malfunction, towards the reservoir, but because no visible abnormalities were noted with the returned components, the complaint could not be confirmed as reported; however, the presence of surface abrasion was noted on the longer length pump exhaust tubing and inlet tubing of the pump.Based on recreation of the position of the tubes according to the abrasion pattern, this demonstrates that the longer length pump exhaust tube was overlapping the inlet tube of the pump. This positioning, in combination with device usage over time, could contribute to sufficient stress to cause a separation through the serialized exhaust tubing at this site.A separation of this type would then allow the loss of fluid, making the device inoperable.A review of the device history record confirmed the devices from this lot met all specifications prior to release and no anomalies were noted during production.A review of the complaint history database, nonconformances and capas revealed no trends for this lot.
