Device evaluated by manufacturer: synergy xd mr us 2.25 x 12 mm stent delivery system was returned for analysis.A visual examination of the stent found that the stent was damaged with struts in the mid-section of the stent lifted. The undamaged crimped stent outer diameter was measured and the result was within maximum crimped stent profile measurement.Stent damage that was noted during analysis most likely occurred during handling of the device during preparation. The balloon cones were reviewed, and no issues were noted. The balloon wings were tightly wrapped and evenly folded and were not subjected to positive pressure.A visual and microscopic examination of the bumper tip showed no signs of damage.A visual and tactile examination of the hypotube found no issues with the hypotube shaft.A visual and tactile examination of the outer and mid-shaft section and a visual examination of the inner lumen found no issues with the extrusion shaft.A recommended 0.014 inch guidewire was tracked through the wire lumen of the device with no issues.Examination of the hub and strain relief via scope did not identify any issues. No other issues were identified during the product analysis.
