Event Description
It was reported that the ventilator kept alarming patient disconnect.The patient was removed from the ventilator and placed on an alternate ventilator.The patient was not harmed or injured as a result of the reported event.Although requested, patient information was not provided.The customer troubleshot with technical support and advised that the circuit had been changed out, however the issue continued.The customer advised that when the ventilator was changed the same mask and circuit were used on the replacement with no issues.The customer was advised to provide what mask as being used to see if the leak settings were set correctly, to check the ventilator diagnostic report and perform a performance verification test (pvt).Upon a follow up, the customer reported that the unit was tested with the original patient settings for well over an hour without issue.The ventilator diagnostic logs were reviewed and had error code patient disconnect multiple times.The reported issue was not duplicated by the customer, the unit passed performance verification and was returned to use.
