The reported device, intended for use in treatment, was not returned to the designated complaint unit for independent evaluation, thus visual inspection and functional testing could not be performed. There was a relationship found between the subject device and the reported incident. The customer provided images confirmed the packaging was contaminated.A review of the device history records showed there were no indications to suggest that the product did not meet manufacturing specification or would not be able to perform as intended.A complaint history review concluded this was an isolated event.A review of the packaging process summary revealed a visual inspection for particulate and seal for breaches is conducted on a sampling plan. The complaint was confirmed and the root cause has been associated with a manufacturing error.Although every effort is made to control particulate in the controlled clean room, it is not possible to completely eliminate foreign matter in the area due to the human element.A correction in the form of a complaint notification has been issued to manufacturing management to mitigate future recurrences.
