H11: section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant/reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.The following were reviewed as part of this investigation: patient severity, complaint and lot history review, applicable previous investigation(s), sample (if available), labeling, applicable manufacture records, and applicable fmea documents.Based on a review of this information, the following was concluded: the complaint of a break is confirmed; however, the exact cause is unknown. One 4 fr single lumen powerpicc catheter was returned for evaluation. An initial visual observation showed use residue on the returned sample. The catheter appeared to have been broken approximately 3.5 cm distal to the molded joint and the catheter was observed to be partially pinched or flattened at its distal tip.The 3 cm depth marker of the catheter near the break site was observed to be faded.A microscopic observation revealed the break site of the catheter was mostly flat and granular on one side and rough and uneven on the other side, which appeared to be pinched into a point. The catheter appeared to be bonded to itself on the side of the break that appeared to be pinched. What appeared to be some abrasive damage was observed on one side of the surface of the catheter leading up to the break site. While the exact cause of the break in the returned catheter could not be determined, possible causes include prolonged and/or repeated circumferential compression and mechanical damage caused by contact with a hard surface or instrument.H3 other text: evaluation findings are in section h.11.
