The results of the investigation are inconclusive since the reported device was not returned for analysis.Based on the information received, the cause of the reported event could not be conclusively determined.The device history record for the reported oad was unable to be reviewed, as the lot number was not provided.Csi id: (b)(4).
 
Event Description
A diamondback peripheral exchangeable orbital atherectomy device (oad) was used to treat a total occlusion in the peroneal artery on low and medium speeds.As the control knob was pulled back, the device became stuck in the vessel and stopped spinning.The oad was unable to be removed.The location at which the device was stuck was identified via angiography, and another access site was created in the leg at that location.The crown was manipulated with an 18 gauge needle and pulled up through the new access site, allowing the driveshaft to be cut and removed via the groin access site.The crown was removed with the needle and the leg was packed and stitched closed.The patient was stable following the procedure and discharged the following day.
