Device evaluation summary: device returned for evaluation.The balloon folds were open on the returned device.The device failed negative prep.Upon inflation the device failed to maintain pressure and liquid leaked from the balloon.Upon inspection a longitudinal burst was visible from the distal cone to the mid-working length.No issues noted with inner member at the glue port or the bifurcate.A 0.014" wire front and back-loaded with no issues.No other deformation was noted.If information is provided in the future, a supplemental report will be issued.
 
Event Description
During a procedure an attempt was made to use one sprinter otw ptca balloon catheter to treat a lesion.Negative prep was not performed.The lesion was not pre-dilated.The device did not pass through a previously deployed stent.It was reported that inflation difficulties occurred during balloon inflation and the balloon does not keep pressure when inflated to 2atm.The patient was reported to be alive with no injuries.
