Pma/510(k) #(b)(4).Device evaluation the zilbs-635-10-6 device of lot number c1736472 involved in this complaint was returned for evaluation, with the original packaging. The packaging was open on receipt. With the information provided, a physical examination and document-based investigation was conducted.Lab evaluation the device related to this occurrence underwent a laboratory evaluation on the (b)(4)2021.On evaluation of the device partial stent deployment (approximately 1.0cm) was observed at the distal end of the outer sheath at the distal white tip.Damage/roughness was evident along the clear section of the outer sheath from the distal tip to approximately 10.5cm from distal tip. There was also evidence of fish mouth damage at the distal tip.The device flushed as expected.A 0.035¿ wireguide could not pass the damage at the distal tip of the device. As the information provided by the customer indicated that the stent partially deployed during the flushing process the additional follow up information was requested following the lab evaluation: did the device make contact with an endoscope/duodenoscope? how did the damage on the outer sheath and distal tip occur? feedback from the customer again indicated that the device had not made contact with the scope.Document review prior to distribution zilbs-635-10-6 devices are subjected to a visual inspection and functional checks to ensure device integrity.A review of the manufacturing records for zilbs-635-10-6 of lot number c1736472 did not reveal any discrepancies that could have contributed to this complaint issue. The review of relevant manufacturing records, confirms the failure mode has not previously occurred with the current lot number.Based on the information available to date, there is no evidence to suggest that there are any manufacturing issues associated with lot number c1736472.There is no evidence to suggest the user did not follow the instructions for use ((b)(4)).Root cause review a definitive root cause could not be determined as the circumstances of use cannot be replicated in the laboratory.Feedback from the customer confirmed that the stent deployed during flushing and that the device did not go down the scope.A possible root cause could be attributed to incorrect handling during the flushing process resulting in the stent partial premature deployment of the stent.Summary complaint is confirmed as the failure was verified in the laboratory.According to the initial reporter the device did not make patient contact.Complaints of this nature will continue to be monitored for potential emerging trends.
