Event or Problem Description
Medtronic received a report that the device failed to detach.It is unknown if another instant detacher was used, or if a manual detachment via hypotube was attempted.The patient's condition necessitating treatment was unknown.The devices were prepared as indicated per the ifu, with the patient's vessel tortuosity and blood flow listed as normal.

Additional information received reported that the several attempts had been made with both the instant detacher and the manual method.There were no issues prior to non-detachment, and there was no damage observed to the pushwire.
