Event Description
|
It was reported that during surgery, when the meniscus mender implant was placed on the table and uncovered for use, it was observed that a loop was fractured.The procedure was successfully completed without significant delay using a back-up device.No patient injury or other complications were reported.All available information has been disclosed.If additional information should become available, a supplemental report will be submitted accordingly.

The reported device, used in treatment, was not returned to the designated complaint unit for independent evaluation, thus visual inspection and functional testing could not be performed.A review of the device history records showed there were no indications to suggest that the product did not meet manufacturing specification or would not be able to perform as intended.A complaint history review concluded this was an isolated event.A review of the part drawing found that the device is specified to be checked for suture strength and sharp edges.A relationship, if any, between the subject device and the reported event could not be determined.Please refer to the instructions for use for recommendations on proper use of the device and potential troubleshooting methods to prevent future reoccurrence of the reported event.If the product associated with this event is returned at a future date, this evaluation will be reopened for investigation.

H3, h6: the reported device, used in treatment, was received for evaluation.There was a relationship found between the returned device and the reported incident.A visual inspection revealed that one of the meniscal suture loops had separated from its handle.The handle was still secured in the packaging, but the suture loop shaft had deformation from improper opening.The device is intended to be unpackaged by pressing behind the handle to release it from the plastic.The other suture loop shaft was bent, and one other instrument in the set was bent to the side.A review of the device records showed there were no indications to suggest that the product did not meet manufacturing specification or would not be able to perform as intended.A complaint history review concluded this was an isolated event.A review of the suture loop found that the 7-strand suture loop is specified to withstand a pull test of 4 pounds minimum.The complaint was confirmed and the root cause was associated with device design.Corrective actions have been implemented for this failure mode.
